COMBO-KEY 

Coaching Ongoing Momentum Building On stroKe rEcovery journeY (COMBO-KEY) Programme

About the programme 

The "COMBO-KEY" programme is: 

  • A new community rehabilitation programme

  •  Designed according to the latest scientific research evidence, combined with the principles of self-management, and the theory of self-efficacy and outcome expectation 

  • Eight weeks duration

  • Led by healthcare professionals and supported by trained volunteers (Coaching Team)

  • Including home visits and telephone follow-up, supplemented by other people with stroke who shares their experience 

  • Assisting people with stroke in mastering self-management skills, building confidence and improving recovery

 

Features of the programme:

l  Providing individualized self-management and rehabilitation guidance for people with stroke

l  Including affected side exercises, medication use, home rehabilitation programme design 

l  Establishing personal rehabilitation goals and action plans

l  Making rehabilitation programmes more realistic

 

"COMBO-KEY" provides a rehabilitation resource kit to help you recover. There are four resources in it: 

  1. Guide to self-management and self-confidence improvement exercises 

  2. Tools and tips

  3. Video sharing of recovery experiences of stroke patients
